Immunization Forms for School or Work

We ask that you come prepared for your appointment with our nurse and complete your portion of the forms to the best of your ability. There is a fee for completion of the forms- please see our uninsured services fees.  As well prior to your appointment with the nurse please ensure you bring the following to your appointment:

Last TB test

Any blood work or serology that has been done previously

Childhood vaccination record (Contact Ottawa Public Health if you don't have your prior vaccination records)
